Mental Health Case Manager -
The Targeted Case Manager will be assisting adult individuals with severe and persistent mental illness. The Targeted Case Manager will provide services to achieve the goals of independence of living, vocational education status, social supports, and reduced hospitalizations. The Targeted Case Manager will be maintaining basic needs and skills such as obtaining housing, food, medical care, recreation, education and employment. The Targeted Case Manager will also offer information, encouragement, support and assurance that help is available to the client and family members.
The Targeted Case Manager shall organize and bring in the natural supports available to the person through family, friends and neighbors, and when the client's needs cannot be met through this means, the Targeted Case Manager shall organize resources available in the local community and given service providers. When the client's needs cannot be met this way, the Targeted Case Manager shall also perform assessment service planning, informal support network building, utilization of community resources, linkage with services, monitoring and delivery of service, outreach and a problem resolution for their clients.
Bachelor’s degree in psychology, social work, nursing, education, or related field. 35 hour workweek with some on call responsibilities Flexible work schedule. Annual salary $42,482.
