At the Family Empowerment Project, we believe in empowering families and improving maternal and infant health outcomes across Brevard County. In partnership with the Healthy Start Coalition of Brevard County, we provide personalized, compassionate support to parents through health education, care coordination, and community connection.

We’re seeking a Healthy Start Home Visitor to support pregnant women and new parents through in-home visits, health education, and resource navigation. You’ll help identify needs, reduce stressors, and empower families to achieve healthy, thriving outcomes.

• Conduct regular home visits with Healthy Start clients
• Provide prenatal and postpartum education and counseling
• Screen for emotional, situational, and developmental risk factors
• Connect clients to local services and resources
• Maintain accurate documentation in our case management system
• Deliver health education on childbirth, breastfeeding, parenting, and more
• Participate in outreach, group education, and community events
• Attend team meetings, training, and professional development activities

• Bachelor’s degree in a health or human services field preferred
• OR Associate degree with 3 years’ related experience
• Bilingual in Spanish or Haitian Creole preferred
• Reliable vehicle, valid FL driver’s license, and auto insurance
• Strong communication, organizational, and documentation skills
• Ability to work independently and flexibly (occasional evenings/weekends)
• Proficient with Microsoft Office and basic data systems
• Must be able to lift up to 25 lbs
• Must pass a Level 2 background check
