Family First Health is seeking a Part-Time Licensed Mental Health Professional with a desire to work with patients 5-100, 20 hours/week position in our Gettysburg and Hanover offices.

The Licensed Mental Health Professional works as a member of a multidisciplinary Primary Care medical team in managing the whole health of Primary Care patients. Offering brief (15-30 minutes), immediate behavioral health support and interventions to primary care patients (5 years of age and older). Provide assessment, evaluation and treatment for primary care patients and offers referrals when appropriate to specialty outpatient mental health services.
Qualifications
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Provides targeted evidence-based assessments and evaluations for behavioral health issues identified within the Primary Care Setting.
- Utilizes the G.A.T.H.E.R model of care
- Generalist: The Behavioral Health Consultant (BHC) works with patients of any age and any behavioral concern, from anxiety to tobacco use or parenting skills
- Accessible: The BHC is available to help the primary care physician at all times during the workday, whether that entails a warm handoff, exchange patient care or provide a curbside consultation.
- Team-Based: The BHC is part of the health care team and participates in medical meetings and huddles about patient care
- Highly productive: High volume of patient care (i.e., 8-10 patients per day; 30 min. sessions)
- Education: The BHC educates patients about health issues and is also a teacher and coach for the rest of the health team regarding patient’s psychosocial needs. The BHC supports the primary care physician to continue care of the patient
- Routine: The BHC becomes a part of the clinic’s normal daily workflow, the BHC is an integrated part of the team and the normalization of behavioral health care destigmatizes the process of working with a behavioral health provider.
- Provides brief, point of care, solution focused interventions to primary care clients to promote well-being and assist with implementation of treatment plans
- Provides skill building, coaching, and psychoeducation to enhance whole health
- Provides assessments to determine appropriate level of care and provide that feedback and recommendations to Primary Care team.
- Provides consultation to patients, Primary Care Providers and other members of the patient care team in a fast-paced environment. Consultations can be both formal and informal.
- Utilize screening and measurement tools to monitor patients response to care
- Provide advocacy and case management services for clients
- Maintains knowledge of community resources and coordinates referrals as appropriate
- Document and maintain electronic health record (EHR) and ancillary records in a manner that assures compliance with all agency policies, program procedures and local, state, and federal regulations. Maintain accurate and up-to-date documentation as required, associated with billing, assessment, treatment, client’s progress and other activities.
- Develop a comprehensive individualized treatment plan collaboratively with patients and Primary Care Provider to address biological, psychological and social aspects of wellness.
- Assist in crisis situations as appropriate, following emergency protocols/procedures coordinating with the treatment team on interventions that support a trauma-informed environment for clients and staff.
- Provide crisis intervention services to patients in order to minimize or eliminate periods of crisis and acute care utilization.
- Requires some travel between offices for various meetings and potential occasional evening work
- Demonstrates an ability to be flexible in workspace, meeting with patients in exam rooms as needed and station oneself in the clinical POD area.
- Meets performance standards for productivity, documentation and adherence to metrics
- Adhere to compliance, required laws and trainings, safety of property and staff, regulations,
- I.e. Mandatory abuse reporting laws, HIPAA, fire regulations
- Participate in all scheduled staff meetings, supervision sessions, and other departmental and agency meetings.
- Complete all annual employee training requirements in order to understand how to conduct ethical work, for professional development, and stay in compliance
- Perform other duties as assigned
Qualifications
- Active Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), or Licensed Marriage and Family Therapist (LMFT) in the state of Pennsylvania.
- Current CPR and Narcan certification
- 2 years of clinical experience
- Experience working in a health care setting, preferred
- Bilingual, preferred
- Experience working with Electronic Health Records (EHRs)
- Knowledge of Trauma Informed Care, Motivational Interviewing, Recovery and Illness Management practices
- Knowledge of computer software applications (Microsoft Office, etc.)

Working Hours are Monday- Friday between the hours of 8:00am – 6:00pm depending upon individual office need.
