\Job Summary
\We are seeking a skilled LPN to provide private duty care to a child with a G-Tube, Trach, and Vent needs. This is a part-time or full-time opportunity (3-4 nights/week, 10pm-7am) available in Chatham, IL.
\Responsibilities
\- Develop and implement individualized care plans for the patient.
- Provide routine care activities such as medication administration, wound care, and vital sign monitoring.
- Maintain accurate and up-to-date clinical records for the patient.
- Collaborate with the multidisciplinary team to address any concerns or issues related to patient care.
Qualifications
\- An active LPN/LVN license in the state of employment or a Multi-State license through the Enhanced Nurse Licensure Compact where recognized.
- A minimum of 1 year of experience in pediatric care.
- A valid driver's license with proof of insurance in applicable states.
- A clear background screening.
- A working knowledge of Nurse Practice Acts.
- A valid and current CPR card/certificate with in-person instructor-led hands-on skills check.
- A clear Hep B Screening in applicable states.
- A clear TB Screening in applicable states.
