Job Description
We are seeking compassionate and skilled Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to join our team as Homecare Specialists. As a key member of our team, you will provide one-on-one patient care, develop meaningful relationships with your patients and their families, and work collaboratively with our interdisciplinary team to deliver high-quality care.
Responsibilities
- Provide direct patient care, including G-tube care, tracheostomy care, and ventilator management
- Develop and implement individualized plans of care, based on each patient's unique needs and goals
- Collaborate with our multidisciplinary team to ensure seamless transitions and continuity of care
- Maintain accurate and timely documentation of patient progress and care delivered
Requirements
- RN or LPN licensure in good standing
- Experience in pediatric home healthcare, preferably with G-tube and tracheostomy care
- Ability to lift, push, and pull up to 25 pounds, with frequent lifting and bending
- Valid driver's license and reliable transportation
