Position Details
The Client Success Manager is primarily responsible for ensuring continuity of care and client satisfaction through effective management and collaboration. This role requires diligent oversight of client needs, coordination with referral partners, and active communication with families to guarantee that the highest standards of service are consistently met.
Job Duties
- Conduct client assessments and evaluations to determine care needs
- Coordinate care plans and services with clients, referral partners and family members
- Provide exceptional customer service and professional care to clients
- Maintain detailed records of client progress and care plans
- Ensure compliance with all relevant regulations and standards
- Act as the primary point of contact for clients and their families, addressing any concerns or issues promptly
- Promote collaborative teamwork among care providers to optimize client care
- Participate in community outreach and networking to build and maintain referral partner relationships
- Must be available for emergency client coverage
- Travel within the greater Spokane/Tacoma/Seattle area as needed or assigned
- Perform any duties as needed or assigned
- Participate in regularly scheduled on-call rotation and handle off-hours staffing needs
Qualifications
- Minimum of 2 years of experience in client management, healthcare coordination, or a related role
- Proven track record of successfully managing client relations and care plans
- Current Certified Nursing Assistant or Home Care Aide Certification required
- Nurse Delegation preferred
- Comply and maintain valid Driver’s License, vehicle insurance with clean DMV record required
- Excellent communication and interpersonal skills
- Strong organizational and time management abilities
- Ability to work independently and as part of a team
- Proficiency in using healthcare management software and tools
- Empathy and dedication to providing high-quality care and service
Compensation: Family First is committed to providing market competitive, equitable pay to all employees
$24.00-$26.00
