What are the responsibilities and job description for the LPN Clinic position at Family Guidance Center?
DESCRIPTION
To organize, manage and provide medical support services to clients receiving rehabilitation services.
EXAMPLES OF WORK
Work may include but is not limited to the following examples:
- Take weights and blood pressures and record results in client records.
- Administer the AIMS to clients on regularly scheduled basis under physician supervision.
- Manage the scheduling of clients to see the physician for medication services and communicate failed appointments to the proper community support worker and/or supervisor.
- Provide education to clients regarding their medication, its benefits and side effects. Document educational sessions.
- Collect and present them to the physician.
- Follow-up with the laboratories regarding client compliance with prescribed laboratory tests.
- Provide injections at physician’s orders.
- Review physician charts.
- Attend administrative and management meetings.
- Conduct metabolic screenings.
- Monitor sample medication supply.
- Continue professional training by attending conferences, workshops and in-service training events.
- Provide ongoing assessment of a client’s status including personal strengths, progress, problems and needs.
- Provide preparation and/or revision of a specific individualized treatment plan.
- Monitor client participation and progress in organized treatment programs to assure the planned provision of service according to the client’s individual treatment plan.
- Provide individual assistance to clients as a twenty-four hour a day resource in accessing needed public services including financial, medical and housing.
- Intercede on behalf of individual clients within the community-at-large to assist the client in achieving and maintaining their community adjustment.
- Must be able to use the agency electronic medical record program for all documentation. EXPERIENCE AND EDUCATION QUALIFICATIONS
The following education and experience qualifications are the minimum requirement used to admit or reject applicants for consideration.
- Hold a current valid license as a practical nurse in MO.
- Have successfully completed training in the provision of injections.
- Prefer at least four years of experience in dealing with individuals who have major psychiatric illnesses.
- Skills and interest in working with chronically mentally ill individuals.
- Possesses a valid driver’s license and is, and remains insurable, under Family Guidance Center automobile liability insurance.

- Documentation will meet agency standards 95% of the time.
