Case Manager/Peer Support Specialist - Springfield

****$1,000 SIGN ON BONUS****

1st $500.00: after 30 days of employment

2nd $500.00: after 6 months of employment

Essential Duties:

• Communicates and assists all patients with their transportation needs which include regular contact with their insurance company and constant tracking and updating of all standing orders.
• Acts as liaison with the MCC) designated transportation companies.
• Facilitates Orientation groups I time per week to review FGC OMT handbook.
• Acts as back up staff member to assist in completion of the Intake Screening for Immediate Access to Care.
• Monitor behavioral compliance issues and report concerns and/or noncompliance to the primary counselor and/or Program Manager.
• Provide standby didactic group services.
• Provide orientation for new patients.
• Assist patients with community resources to obtain medication when necessary.
• Plan and coordinate sober leisure activities for all patients.
• Conduct group didactic instruction, and case management support of the primary counselor

Work Schedule: Must work a minimum of 40 hours per week and available for Saturday, opening and closing (if needed). Monday-Friday 8:00am-4:30pm (can vary as based on needs of patients). Hours may vary based on program needs of agency and flexibility is required.

Work Location: 120 N. 11th St, Springfield, IL

Pay Rate: Up to $19.23 per hour; based on experience

Experience and Skills:

Education and/or Experience: A High School diploma or equivalent is required. Have a minimum one year of recovery from substance use, mental health, or co-occurring substance use and mental health challenges.

Certificates, Licenses, Registrations: Certified by the Illinois Certification Board (ICB, Inc.) as a Certified Recovery Support Specialist (CRSS) or as a Certified Peer Recovery Specialist (CPRS) or be certified within one year of employment date.
