Immediate Access to Care Coordinator - Manteno

****$1,000 SIGN ON BONUS****

1st $500.00: after 30 days of employment

2nd $500.00: after 6 months of employment

Essential Duties and Responsibilities: Provides coordination for all patients seeking treatment services at Family Guidance Centers, Inc. and screening for all levels of care and assists with the coordination of care.

• Answers daily inquiries from patients and referral sources via phone and assists patients with placement in treatment services according to data obtained from screening based on ASAM criteria.
• Assists in performing comprehensive psychosocial assessments with patients in order to determine appropriate level of care and placement when needed.
• Make appropriate referrals for other community program services when necessary to assist patient.
• Coordinate with Access to Care Coordinators and Access to Care Program Manager for immediate access to treatment services based on availability.
• Treats patients with respect and dignity and maintains and respects the confidentiality of all patients in accordance with 42 CFR and HIPAA regulations. Maintains a professional demeanor at all times.

Schedule: Monday - Friday (8:30a - 5:00p), with a rotating Saturday

Start Date: ASAP

Work Location: 333 W. Evergreen St., Manteno, IL

Pay Rate: $17.25 - $20.00

Experience and Skills:

Education and/or Experience: High School Diploma / GED required; Experience in/with substance use, detox programs, medication-assisted treatment, behavioral healthcare and/or community relations encouraged to apply.

Certificates, Licenses, Registrations: IAODAPCA Certified Alcohol and Other Drugs Counselor (CADC) required, or able to obtain within allotted time of hire date.
