Essential Duties
Work Location: 1220 S. Wood St., (U of I Mile Square Health Center), Chicago, IL 60608
Schedule: Monday - Friday (6:00a - 2:30p, with flexibility) and rotating Saturdays; flexibility is required.

Required Experience
Education and/or Experience: Bachelors degree (or higher) and prior substance abuse treatment experience preferred. Will consider relevant internship experience in lieu of paid employment.
Certificates, Licenses, Registrations: IAODAPCA Certified Alcohol and Other Drugs Counselor required; given two years to acquire upon hire date or based on previous substance abuse position experience. State of Illinois Licensed Clinical Professional Counselor, Licensed Professional Counselor, Licensed Social Worker or Licensed Clinical Social Worker preferred.

Essential Duties
- Completes SAMMS intake assessments for new patients
- Completes an individualized treatment plan for each patient on caseload addressing all needs identified in the psychosocial assessment
- Provides group and individual sessions to patients in accordance with their treatment plan
- Properly documents all services provided in progress notes
- Reviews and evaluates patients progress and continued stay in treatment in relation to measurable goals and objectives described in treatment and care plans
- Modify treatment plans to comply with changes in patient status
- Plans and assists with the implementation of aftercare and continuing recovery plans
- Works with patient and other staff to deescalate and assist with patient emergencies and crisis situations, as needed
- Coordinates care with other treatment providers (i.e. DCFS, probation/parole, and others as necessary)
- Participates in any treatment team staffings.
- Makes appropriate patient referrals to outside agencies, as needed
- Assists with the collection of toxicology specimens, as needed.
- Attends in-house and off-site trainings for continuing education, in order to expand knowledge, improve the quality of patient services, and to maintain licensure and/or certification at all times.
