Essential Duties:
- Develop patient treatment plans and conducts continued stay review based on research, clinical experience, and patients’ histories - using SAMMS assessment on computer.
- Provide patient intakes (biopsychosocial assessment), treatment planning, case coordination, staffing, aftercare planning and discharge with individual and group therapy for a full spectrum of referral sources.
- Provide direct clinical service to a full caseload, achieving a minimum of 20 billable hours weekly through both individual and group counseling sessions.
- Coordinate patient care with UI Health Primary Care Physician, Nurse Practitioner, FGC Nurse and FGC Recovery Support Specialist/Case Manager through a collaborative approach.
- Present each patient at weekly multidisciplinary staffings and report progress toward treatment goals as well as identify current challenges and barriers to treatment.
Work Location: 1220 S. Wood St., (U of I Mile Square Health Center), Chicago, IL 60608
Schedule: Monday - Friday (6:00a - 2:30p, with flexibility) and rotating Saturdays; flexibility is required.
Start Date: ASAP
Pay Rate: $42,000 - $45,000

Experience and Skills:
Education and/or Experience: High School Diploma / GED required; Experience in/with substance use, detox programs, medication-assisted treatment, behavioral healthcare and/or community relations encouraged to apply.
Certificates, Licenses, Registrations: IAODAPCA Certified Alcohol and Other Drugs Counselor (CADC) required, or able to obtain within allotted time of hire date.
Salary : $42,000 - $45,000
