POSITION SUMMARY :
As an outpatient therapist you will conduct assessments, develop treatment plans and provide individual, group or family therapy sessions
As both a therapist and consultant you will assists individuals and groups in addressing their Behavioral Health and Substance Use disorders which may include personal, social, emotional and environmental difficulties which predispose illness or interfere with them entering recovery for substance use disorders / mental illness or chronic medical condition.
In the role of Consultant, therapist will work in an integrated team-based model of care to improve patients' health literacy, engagement, psychosocial functioning, compliance, and overall functioning and wellbeing.
DUTIES AND RESPONSIBILITIES :
Performs clinical assessments and evaluations with patients to determine functioning and an appropriate level of care
Diagnosis and treat mental health disorders
Create individualized treatment plans according to patient needs and circumstances
Meets with patients regularly to provide counseling, treatment and adjust treatment plans as necessary
May conduct evidenced based group for conditions such as Substance Use Disorders, Chronic Pain, Children's Issues, Anxiety, etc.
Conducts brief screenings to evaluate the needs of adults, adolescents, and children for behavioral health intervention, activation
Provides consultation to medical providers, Family Health Center staff, and other service providers to facilitate a coordinated continuum of care within the primary care setting
As part of an integrated team provides short-term behavioral intervention to engage and motivate clients to actively manage their care
Applies knowledge of evidence-based practice, including empirical bases of assessment, intervention, and other clinical applications, expertise and patient preferences to individuals, couples, groups and families to include adults, children and adolescents as assigned
Provides crisis support and referral for evaluation for psychiatric hospitalization if needed.
Creates and implements integrated treatment plans, ensuring access to organizational and external services within allotted time frames
Understands the role of Trauma Informed Care and communicates with team members and physicians about the impact of trauma on the patient's well-being and potential concerns
Communicate with health care team members and agencies to obtain pertinent information to be utilized in evaluating patient needs
Prepares case histories for use in diagnosis and treatment. Develops intervention and treatment plans which include an evaluation of family and community support, patient strengths, and level of risks.
With patient permission works in close collaboration with physicians and other health care personnel in patient evaluation and treatment to further their understanding of significant social and emotional factors underlying patient's health problem
Helps patient(s) through individual or group sessions to understand, accept, and follow all recommendations. Follows through on treatment plan in patient's records
As a consultant provides direct intervention with patients and families in areas like mental health, suicide, domestic violence, sexual assault, death / grief, crisis intervention, advocacy, substance use, health education and intervention strategies
Makes referral for additional ancillary services as clinically appropriate.
Counsel patients and families on available community resources and supports.
Maintains required case documentation and records delivered services in an accurate and timely manner as required by regulating entities, accreditation organizations, and management.
Attends required in-service, departmental and organizational meetings. Participates in clinical review of cases

TYPICAL WORKING CONDITIONS :
Work is performed in a clinical setting. Contact with patients and staff.
Exposed to communicable disease, toxic substances, medicinal preparations, and other conditions common to a clinic environment.
Requires working under stress, in emergency situations or during irregular hours.
The noise level in the work environment is usually quiet but occasionally may become disruptive while working with a client.
TYPICAL PHYSICAL DEMANDS :
While performing the duties of this job, the employee is regularly required to use hands to finger, handle, or feel and must be able to communicate by talking or listening.
The employee is frequently required to stand; walk and sit. The employee is occasionally required to stoop and kneel.
The employee must occasionally lift and / or move up to 25 pounds.
Specific vision abilities required by this job include close vision, peripheral vision and ability to adjust focus.
QUALIFICATIONS :
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and / or ability required. Requires Master's in Behavioral Health Field (Counseling Psychology, Social Work) and licensure LMSW, LLMSW, LPC Licensure. Experience with mental health and substance abuse is desirable.
