What are the responsibilities and job description for the Nutritionist position at Family Health Care Clinic, Inc.?
Training and Qualifications: Dietitians and nutritionists need at least a bachelor’s degree in dietetics, foods and nutrition, food service systems management, or a related area. Two years of experience preferred.
Certify and recertify women and children for the WIC program Provide Individual and Group Nutrition Counseling.
Coordinate and/or integrate care across all elements of the complex health care system (e.g., subspecialty care, hospitals, home health agencies, nursing homes) and the patient’s community (e.g., family, public and private community-based services).
Care is facilitated by registries, information technology, health information exchange and other means to assure that patients get the indicated care when and where they need and want it in a culturally and linguistically appropriate manner.
Advocate for patients to support the attainment of optimal, patient-centered outcomes that are defined by a care planning process driven by a compassionate, robust partnership with patients, and the patient’s family.
Incorporate Evidence-based medicine (Clinical Measures) and clinical decision-support (CDSS) tools guide decision making. Accept accountability for continuous quality improvement through engagement in performance measurement and improvement.
