Family Health Care is currently accepting applications for CAHC Clinical Patient Navigator in our Evart Schools location!
The CAHC Clinical Patient Navigator position provides direct support and assistance with patient care services under the direction of clinical leadership with Family Health Care's Childhood and Adolescent Health Center (CAHC). This position is responsible for the following CAHC Clinical Patient Navigation duties; Medical Assistant, Medical Office Assistant, Referrals, Medical Records, Medicaid Enrollment and Outreach Services.
Primary Responsibilities
- Greets and acknowledges all patients that come to the front desk window or are in the front lobby/patient areas.
- Consistently provides efficient and accurate scheduling of patients according to provider/department specifications.
- Routinely completes pre-visit chart preparation and manages the assigned clinic schedule as well as the daily flow of patients while also assisting the medical provider in seeing scheduled and walk-in patients.
- Obtains subjective and objective data for services according to FHC/CAHC policies, procedures, and standing orders. Reports significant findings to provider and documents in patient EHR.
- Assists the provider in the administration and recording of medications, treatments, and completes screenings as ordered by the provider and in accordance with CAHC standing orders, policies, protocols and procedures.
- Completes all provider orders/tasks to finalize the patient visit.
- Provider orders, patient plan, discharge instructions.
Educational Requirements
Medical Assistant - Certified or Registered
Location: Evart, MI
Exempt/Non-Exempt: Non-Exempt
