The Senior Outreach Coordinator/LPN plays a vital role in both the medical and community health departments. This position involves providing outreach support for various grants, contracts, and programs that serve the community, as well as conducting comprehensive annual wellness health assessments for Patient-Centered Medical Home (PCMH) Medicare patients at the Baldwin Clinic.
The Senior Outreach Coordinator/LPN is responsible for assessing, planning, implementing, coordinating, monitoring, and evaluating services to optimize patients’ health statuses. They consistently adhere to program requirements and guidelines while promoting quality and efficiency in community care delivery.
In the clinic, the Senior Outreach Coordinator/LPN ensures that BFHC’s Medicare patients receive their Medicare Annual Wellness Visits (AWVs) during scheduled appointments. They also strictly follow standing orders, policies, and procedures in both the clinic and community settings.
