What are the responsibilities and job description for the Behavioral Health Provider position at FAMILY HEALTH CENTER OF SOUTHERN OKLAHOMA?
JOB SUMMARY:
As an integral member of the provider team, provides direct behavioral health services to patients under direction and responsibility of a licensed psychologist/licensed psychiatrist/licensed clinical social worker.
RESPONSIBILITIES:
- Provides direct patient care, performs comprehensive examination, obtains a thorough and appropriate behavioral health history, compiles patient data and arranges testing if needed, therapy, and follow-up for behavioral health patients of all ages.
- Be responsible for initial behavioral health evaluations.
- Complete paperwork as required by statute, regulation and/or FHCSO Policy and Procedures.
- Develops and implements patient management plans, records progress notes, and assists in provision of continuity of care for behavioral health patients.
- Instructs and counsels patients regarding compliance with prescribed therapeutic regimens, emotional problems of daily living.
- Completely documents each patient’s encounter clearly and concisely, on the day of service.
- Maintains up to date required licensure and credentials.
- Initiates patient referrals as necessary, and communicates with the referral clinician or agency.
- Support activities related to the Health Disparity Collaborative. Duties include but are not limited to supporting the incorporation of staff time for data entry, team meetings, production and submission of monthly reports, maintenance of database.
- Maintains and improves skills by attending health center provider meetings.
- Consults medical physician when appropriate.
- Participates in community agencies and/or resources in providing continuity of care.
- Participates in designated activities and projects for PCMH as assigned by the Chief Medical Officer.
- Follows the mission and vision statement.
- All other duties as assigned.
