ESSENTIAL DUTIES AND RESPONSIBILITIES
The essential functions may include, but are not limited to the following:
• Complete provided assessments of assigned clients within policy timeline.
• Develop and complete Person-Centered Treatment Plans utilizing appropriate assessments and treatment plan reviews with clients as identified by policy.
• Provide services to clients as identified through the treatment plan and assessments with materials provided by FHC.
• Complete Electronic Medical Records (EMR) documentation within 24 hours per policy, including but not limited to progress notes, State Registration, and other documents.
• Document all client contacts including in person, by phone and with the client.
• Through training, become competent in completing state registration as assigned and as scheduled.
• Participate in team meetings, communicate, and cooperate with both internal and external team members regarding tasks, and contribute actively to the team’s efforts to ensure the client’s needs are met.
• Maintain a minimum of a 2-week schedule with clients, in EMR.
• Participate in staff meetings, consult with team members of shared clients, and attend appropriate client appointments.
• Follow designated curriculums when conducting skills training individually or in a group.
• Maintain knowledge of Medicaid Rehabilitation Option (MRO) guidelines provision of services, documentation requirements, qualifying diagnoses, and level of need issues.
• Follow expectations set forth by the Family Health Center in trainings, productivity, policies, and documentation.
• 10%, Other duties as assigned.
MINIMUM QUALIFICATIONS (KNOWLEDGE, SKILLS, AND ABILITIES)
• Minimum Education: Bachelor’s degree in a behavioral health field
• Minimum Experience: 2 years’ experience in behavioral health, addictions, community resourcing, preferred but not required
• Minimum Knowledge and Skills:
• Must be 23 years old or older
• Knowledge of Medicaid, Medicare, and other insurance helpful,
• Must be able to drive self and clients safely
• Have positive communication skills
• Have the ability to maintain confidentiality
• Ability to work with clients from diverse backgrounds
•Sensitive and flexible when dealing with clients
