What are the responsibilities and job description for the Wraparound Facilitator position at Family Health Center?
Position Summary
The Wraparound Facilitator is responsible for following the state model of high- fidelity wraparound process with families & working in full partnership with the wraparound team members to develop a plan of care, oversee the implementation of the plan, identify formal and non- formal strategies to assist families with meeting their underlying needs. The Wraparound Facilitator assures that care is delivered in a manner consistent with strength-based, family centered, and culturally competent value. The Wraparound Facilitator offers education to all team members regarding the values of the process while monitoring progress towards meeting the underlying needs in addition to ensuring all documentation is completed based upon the wraparound program in which the youth qualifies.
Essential Job Duties
1. Work with families towards completing the four phases of the Wraparound process through the development of wraparound teams and the establishment of a comprehensive plan of care.
2. Document all interactions with each family within the electronic medical record as well as under the wraparound program system in which the youth qualify.
3. Develop and maintain positive relationships with youth, caregivers and teams in which the wraparound facilitator partners with.
4. Work with the Division of Mental Health and Addiction under Children’s Mental Health Wraparound as well the Department of Child Services Children’s Mental Health Initiative to ensure all pieces within the process are met with the high- fidelity model.
5. Complete all trainings as outlined by DMHA with the completion of additional workshops, boosters and trainings as required.
6. Follow the Family Health Center/ DMHA expectation of ratio of family to wraparound facilitator partnerships.
7. Other duties as assigned.
Job Specifications:
- Bachelor’s degree from an accredited college or university in social work, psychology, or related field.
- 2 years of experience working with children and families.
