Job Summary
This role involves providing case reviews to patients within assigned community health programs, ensuring that the medical program is clinically correct and being followed. The ideal candidate will have experience working in community health and providing human services to medically underserved populations.
Key Responsibilities:
- Conduct thorough case reviews to ensure that patients receive the best possible care.
- Complete accurate and timely documentation according to department standards.
- Ensure that all client files are up-to-date and signed by the RN monthly.
- Perform regular chart reviews to ensure compliance with medical programs.
- Review and sign off on all Case Managed client Care Plans, Initial Assessments, and Comprehensive Assessments.
Requirements:
- Current California Registered Nurse licensure;
- Nurse Practitioner, Physician Assistant, or Medical Doctor licensure accepted;
- Or equivalent combination of education and experience that provides the skills, knowledge, and ability to perform the essential job duties;
- 1 year of experience in community health and in providing human services to medically underserved, high-risk, and/or disabled populations;
- Ability and means to travel as needed promptly within San Diego County, to locations that may have limited access to public transportation;
- Proof of liability and property damage insurance on vehicle used is required.
Preferred Qualifications:
- Experience working with diverse populations.
- Strong communication and interpersonal skills.
- Ability to work effectively in a team-based environment.
- Basic computer literacy.
- Bilingual in English/Spanish preferred.
