Job Summary

Responsibilities:
- Provides case reviews to patients within assigned community health programs, ensuring the medical program is clinically correct and being followed.
- Completes all required documentation accurately, promptly, and thoroughly by department standards;
- Ensures that all client files are kept up to date with RN signature on required documents every month;
- Performs chart reviews, ensuring the medical program requested is being followed;
- Ensures applicable regulatory and department standards are adhered to on an ongoing basis;
- Reviews and signs off on all Case Managed client Care Plans, Initial Assessments, and Comprehensive Assessments.
Requirements:
- Current California Registered Nurse licensure;
- Nurse Practitioner, Physician Assistant, or Medical Doctor licensure accepted;
- Or equivalent combination of education and experience that provides the skills, knowledge, and ability to perform the essential job duties;
- 1 year of experience in community health and in providing human services to medically underserved, high-risk, and/or disabled populations;
- Ability and means to travel as needed promptly within San Diego County, to locations that may have limited access to public transportation;
- Proof of liability and property damage insurance on vehicle used is required.
