**Job Description**
We are seeking a licensed therapist to join our team at Family Health Centers of San Diego. As a member of our multidisciplinary team, you will work closely with case managers, patient support staff, and other medical professionals to provide high-quality care to our patients.
Your responsibilities will include:
- Performing initial assessments and ongoing care
- Participating in the development of individualized care plans
- Coordinating with case managers and patient support staff
- Providing education and support to patients and their families
**Requirements**
To be successful in this role, you will need:
- An active registration in good standing with the State of California as a Licensed Professional Clinical Counselor (LPCC), Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), or Clinical Psychologist
- A master's degree in counseling, social work, or a related field
- Experience working in a community health setting, preferably with underserved populations
- Excellent communication and interpersonal skills
