POSITION DESCRIPTION:
Responsible team member for the overall services of the addiction counseling services needs of FHC patients. This team member is responsible for conducting clinical evaluation; treatment planning; addiction counseling; case management, and referrals under clinical supervision. The team member also provides leadership in program planning and problem-solving. Is responsible for delivering substance use counseling services to Family Health Centers patients. Consults with medical providers to ensure substance use counseling services are integrated into their medical care. Adheres to the Corporate Compliance Program by reporting improper or unethical conduct and violating applicable laws, regulations, or program requirements.
DETAILED DUTIES AND RESPONSIBILITIES:
- Provide assessment and evaluation, treatment planning, referrals, service coordination, and patient education.
- Must be able to follow the Suboxone Program Protocol and work alongside the provider
- Review and evaluate patient progress related to measurable goals described in treatment and care plans.
- Maintains accurate data and reports and, in cooperation with other Management Team members, assists in compiling reports such as the UDS and the clinical activity report and, through the middle management, coordinates the data collection necessary for these reports
- Maintains accurate documentation of the client’s treatment progress following agency, local, state, and federal standards.
- Directly consult with medical staff concerning Substance use health issues as needed.
- Process referrals from providers and the Referrals Department for substance use health needs and document the outcome.
- Use EHR to document services rendered and chart all encounters.
- Attend community functions and meetings and provide substance use outreach service as required.
- Consult with supervisory staff concerning programmatic concerns and policies.
- Assesses and track the client’s stages of change readiness and stages of recovery.
- Provides supportive therapy and psycho-educational therapies to clients in individual, group, and family settings in the office and community setting.
- Collaborates with other team members to develop and implement a client-centered treatment plan.
- Ensures proper program components to meet licensing and, or accreditation standards.
- Acts as a liaison between the patient and selected community agencies as determined by the social services director to coordinate health care needs/ referrals for patients with substance use.
Requirements
KNOWLEDGE, SKILLS, AND ABILITIES:
- Experience working with adults with co-occurring disorders.
- Must have interpersonal skills sufficient to establish counseling relationships with clients and to communicate and work effectively with others involved in the client’s treatment.
- Ability to work well with an interdisciplinary team
TRAINING AND EXPERIENCE:
- Bachelor’s Degree in a Social Work/Human Services field is preferred.
- State of Florida Certified Addictions Counselor (CAC) certification will be obtained within six months.
- Minimum one year experience as a Substance Use/Addictions Counselor.
SPECIAL REQUIREMENTS:
- Bilingual skills in English/Spanish/Creole preferred.
