What are the responsibilities and job description for the Home Health Coordinator/ Scheduler position at Family Home Care Group?
Qualifications
- Screening new referrals to be accepted from different portals (Navihealth, Allscripts, Aidin)
- Obtaining authorization for homecare services from different Insurance companies
- Verifying insurance eligibility and accepting cases from insurance companies and managed care companies
- Request home health orders from primary care physicians, hospitals, nursing homes, and other healthcare facilities.
- Work closely with nurse case managers to accept referrals.
- Process new referrals from MD offices, hospitals, nursing homes, and other healthcare facilities.
- Coordinate activities of orders, intake, evaluation of admission criteria, referral/authorization of cases, and transition of care for the Home Health Agency.
- Perform other duties as required.
- Accept referrals via different portals and input them into the pending list while waiting for authorization or additional orders to staff them.
- Must possess excellent communication skills, the ability to interact well with a diverse group of individuals, strong organizational skills, and the ability to manage and prioritize multiple assignments.
- Must have at least 1 year of home health experience.
- Coordinator is directly responsible for scheduling visits and communicating with field staff, patients, physicians, etc to maintain proper care coordination and continuity of care.
