Key Responsibilities:
- Conduct assessments to determine clients' psychosocial needs and care requirements.
- Develop and implement care plans in collaboration with interdisciplinary teams.
- Provide counseling and support to clients and their families, addressing emotional and social challenges.
- Advocate for clients' needs within the healthcare system and connect them with community resources.
- Document and maintain accurate records of client interactions and progress.
Required Skills and Qualifications:
- Master’s degree in Social Work (MSW) from an accredited program.
OR Valid state licensure (LCSW or equivalent) to practice in home health settings.
- Strong interpersonal and communication skills to effectively engage with clients and families.
- Experience in home health or a related field, with a focus on geriatric care preferred.
- Ability to work independently and as part of a collaborative team.
Qualifications:
- Master's degree in Speech-Language Pathology from an accredited program.
- State of Florida licensure to practice as a Speech-Language Pathologist.
- Previous experience in a home health setting is a plus.
- Strong communication and interpersonal skills.
- Ability to work independently and manage time effectively.
- Current CPR certification required.
- Must have a current driver's license and vehicle insurance.
Work Environment:
- Visiting patients in their homes, which may require local travel.
- Flexible hours to accommodate patient needs.
