What are the responsibilities and job description for the New Patient Intake Specialist position at Family Home Care Services of Brooklyn and Queens?
ESSENTIAL DUTIES AND RESPONSIBILITIES :
- Communicates with MLTCs, potential private clients and / or payers regarding the intake of prospective clients
- Verifies insurance eligibility when applicable
- Enters information in the system as necessary to ensure accurate and complete client information
- Communicates with internal staff regarding any pertinent information of new clients
- Issues reports and maintains record keeping of all leads
- Generates reports for the weekly New Business meetings
- Has knowledge of and adheres to all Company policies associated with data integrity, maintenance and confidentiality.
- Coordinates record maintenance and submission as requested for audits.
- Enters Compliance information in the system for newly hired aides
- Assists the Director of Coordination and New Business in developing community education plans
- Maintain excellent relationships with all plans and providers
- Keep track and submit to Senior Management records of YTD activity when requested
- Other duties as assigned
