Physical Therapist Opportunity
We are seeking a highly skilled Physical Therapist to join our dynamic team at Family Home Health Network. As a Physical Therapist, you will have the opportunity to provide high-quality care to patients in their homes throughout the Chicagoland area.
About the Role:
- Provide physical therapy services to patients, including assessments and evaluations to determine therapeutic, rehabilitative, and functional status
- Develop and implement individualized plans of care to meet patient needs
- Instruct patients, families, and caregivers on the use and care of therapeutic appliances
Qualifications:
- Baccalaureate or master's degree in physical therapy from an accredited program
- Current Illinois license as a Physical Therapist
- Current CPR certification
- Valid IL driver's license and auto insurance policy
