Key Responsibilities:
- Assess and evaluate patients' therapeutic, rehabilitative, and functional status
- Develop and implement individualized plans of care in collaboration with the healthcare team
- Conduct physical therapy sessions to improve mobility, strength, and overall well-being
- Instruct patients, families, and caregivers on proper use and care of therapeutic appliances
Qualifications
To be successful in this role, you will need:
- Baccalaureate or master's degree in physical therapy from an accredited program
- Current Illinois licensure as a Physical Therapist
- CPR certification
- Valid Illinois driver's license and automobile insurance policy
- Prior experience in home health care is preferred but not required
