What are the responsibilities and job description for the Triage Hospice Nurse position at Family Home Health and Hospice?
Family Home Health Network, is a provider of intermittent Home Health care, Palliative care, Hospice care and Skilled/Custodial private care services throughout the Chicagoland area.
We are seeking a dedicated Hospice Triage Nurse with a strong commitment to quality patient care. To succeed in this role, you must possess excellent clinical nursing and customer service skills. This position starts Friday at 5pm and lasts until Monday at 8am.
Qualifications:
- Must possess current Illinois RN licensure
- Possess a minimum of two (2) years experience (Hospice)
- Current CPR
- Home Care Home Base experience a plus
- Must possess excellent clinical nursing and customer service skills
