We are looking for a full time Case Manager to join our RISE department.

Summary: Case Managers will assess patient needs and work with clinical teams to develop a treatment plan, choose necessary resources for each patient's needs, and set up referrals.
Education: Bachelor’s degree in social work and/or Masters in psychology or related field of study or LCSW preferred. Must have education deemed necessary by joint commission, CMS and/or applicable State Regulations to properly provide case management services
at FHMC facilities.
Licenses/Certification: Must hold certifications and or licenses deemed necessary by joint commission, CMS, and or applicable State regulations to properly supply
case management services at FHMC facilities.
Experience: Three years of case management experience working with substance abuse patients is preferred.
Duties
- Observing and assessing clients' behavior.
- Developing and implementing treatment plans to address substance abuse and behavioral issues.
- Supporting clients throughout the treatment process.
- Communicating with clients' caregivers, and families to implement treatment plans.
- Monitoring and recording treatment progress.
- Conducting assessments to determine clients' improvements.
- Maintaining client records and treatment reports.
- Collaborating with other healthcare specialists and support organizations.
