Salary : starting salary $80,000 - $105,000
Monday thru Friday 8 : 30 AM 5 : 30 PM or 40-hour week. May require evening and Saturday hours.
INTRODUCTION
Under the supervision of the Medical Director, the Nurse Care Manager works with diverse populations with multifaceted needs. Primary responsibilities include, but are not limited to, assisting clients in following through on established case plans, addressing social determinants of health, enhancing social functioning, and improving the clients quality of life. The Nurse Care Manager serves as planner, advocate, and record keeper for the client.
MAJOR DUTIES / ESSENTIAL FUNCTIONS
Essential and other important responsibilities and duties may include, but are not limited to the following :
- Educating, training and management of medical assistants to perform all necessary task.
- Triage of walk-in appointments
- Participate in Quality improvement measures within the Organization.
- Point of Contact of Vaccination programs : VFC, responsible for vaccination clinics
- Emphasize continuity of care, thus reducing or eliminating fragmentation, duplication, and gaps in treatment plan.
- Act as a patient advocate, protecting privacy and confidentiality issues.
- Provide patient education, monitoring of health needs, and coordination of community resources.
- Initiate appropriate evaluation and emergency management for emergency situations (i.e. cardiac arrest, respiratory distress, injuries, burns, hemorrhage).
- Management of clinic flow and needs
- Attends administrative meetings
- Performs other duties as assigned.
