What are the responsibilities and job description for the Health Hub Navigator position at Family Medical Center of Michigan?
Family Medical Center of Michigan has a full time opportunity for a - Health Hub Navigator, located within Detroit Public School locations.
The Health Hub Navigator works onsite at a Detroit Public School Health Hub. They are a community resource expert, working directly with students and families in the school district and provide referrals to community resources that improve student and family health, economic stability, and security to improve academic outcomes. and referral tracking to community resources that will help improve academic outcomes for students. The Health Hub Navigator coordinates with other Health Hub staff, Detroit Public School Community District staff and neighborhood feeder schools to ensure students and families have access to resources.
Knowledge, Skills and Abilities the ideal candidate must have:
· Must be well-organized, detail-oriented, and can multi-task in a demanding and constantly changing environment
· Must be proficient with MS Office including MS Excel and Word
· A strong commitment to serving the underserved and vulnerable populations of the local community.
· Strong interpersonal communication skills and the ability to communicate easily with others, including demonstrating active listening skills
· Ability to navigate the health care system and advocate for others
· Display empathy, respect, and understanding of community resources, clinical core measure goals, and understanding of health center’s values and processes
· Ability to maintain confidentiality
· Experience in community work, education, or health care strongly recommended
· Bi-lingual a plus
Qualifications:
· HS Diploma/GED, preferably an Associates or bachelors degree in related field
· Community Health Worker / CHW or similar experience
· One year experience in a similar role or other experience/education in a school based position.

Pay: $19.69 - $22.00 per hour
