What are the responsibilities and job description for the Case Manager position at Family of Woodstock, Inc.,?
CASE MANAGER – Family House is the only Teen Shelter in Ulster County, NY. The ideal candidate will have previous RHY / supervisory experience. They should be a positive role model who our young people (12-17 years old) can look up to and who their loved ones and our community partners can feel confident working with. The hiring for any position at Family House is contingent upon a few factors. For example, whether the candidate’s availability is flexible and beneficial to the program’s needs and/or how one candidate's experience might fit more than another's.
General Responsibilities:
- Provide adequate supervision of youth, encourage education, and guide residents through goal setting and structured programming.
- The Case Manager will help the young person establish meaningful and attainable goals, while referring to the program's new level system and help to celebrate the young person's big and small accomplishments.
- Maintain professional and supportive relationships and communicate with all providers, schools, parents/guardians responsibly.
- Actively participate with case conferences and provide aftercare services. Help to facilitate family meetings that focus on reunification efforts.
- Offer meaningful observation and handle data entry in a detailed and timely manner.
- Help the youth feel welcome, accepted for who they are, and connect with a caring community as a pathway towards becoming future leaders.
- Assist the youth to identify their individual strengths and skills. Family House is determined to motivate residents to rediscover their value as a person, despite their circumstances.
- Transport youth when necessary and/or assist the program with basic house chores.

The Case Manager may lend supervision to Residential Counselors when necessary, and/or assist with on-call responsibilities. The Case Manager may also assist with the scheduling of various enrichment activities (workshops) related to academic, recreational and/or personal growth. The ideal candidate will have a Bachelor's Degree (minimum) in Human Services and/or relevant and commensurate experience. Our agency does offer fully paid health (dental, vision, life) premiums, as well as ample paid vacation and sick time. (For e.g., four weeks of vacation, eleven holidays, and two weeks of sick time). We also offer plenty of training opportunities to help compliment a person's professional / lived experiences. Applicants must have a valid driver’s license, dependable transportation, a yearly health statement, background clearances from the State Central Registry and the NYS Justice Center, as well as four references (2 professional and 2 personal) when interviewed. Bilingual is helpful but not necessary.
