**Job Summary:**
The Direct Care Worker (DCW) will provide habilitative assistance and care to clients in various Agency programs, including community and supported living arrangements, family support services program, and day/vocational program. Responsibilities include direct daily care, teaching, supervising clients, and record-keeping.
**Responsibilities:**
- Supervise clients ensuring their well-being and knowing their status and location at all times.
- Assist in developing and implementing individual client plans and educational goals.
- Teach, supervise, and assist clients in daily living activities such as personal care, meal planning, community participation, and more.
- Monitor client progress on goals and protect client rights, reporting any incidents of abuse or neglect.
- Complete incident reports, medication error reports, and implement behavioral plans.
- Collaborate with therapists and other professionals to implement recommendations and programs.
- Organize recreation and community activities for clients.
- Maintain accurate record-keeping of services provided and client goals.
- Attend meetings, trainings, and maintain required certifications.
- Administer medications, provide health care, and first aid to clients.
- Keep program sites and equipment clean and organized.
- Transport clients to activities and programs.
- Participate in emergency drills and ensure safety protocols are followed.
- Perform other related duties as assigned.
**Qualifications:**
- Successful completion of a Medication Certification Program.
- Completion of PMT, CPR, and First Aid training.
- Valid CT driver's license with a satisfactory driving record.
- High School Diploma or equivalent.
- Minimum of two years of experience working with individuals with developmental or behavioral challenges preferred.
**Physical Demands:**
- Ability to lift up to 75 pounds and exert force when necessary.
- Good vision and ability to adjust focus.
- Ability to stand, walk, sit, handle objects, and reach with hands and arms.
**Language Skills:**
- Ability to read and interpret English documents and write reports in English.
**Mathematical Skills:**
- Ability to perform basic mathematical calculations.
**Reasoning Ability:**
- Capable of applying common sense and problem-solving skills.
**Work Environment:**
The position requires traveling by automobile and exposure to changing weather conditions. The noise level in the work environment is usually moderate.
