JOB DESCRIPTION

Classification: Non-Exempt Job Status: Reports to: Director of Nursing

JOB SUMMARY:

The Care Team LPN/RN will work in conjunction with the Primary Care Providers and the nursing department in order to assess, plan, implement, coordinate, monitor, and evaluate patient care for qualifying patients.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Regularly and predictably attend work.
  • Utilize scheduling system to add patients to the appointment schedule
  • Manage incoming and outgoing communication via phone, mail, fax and internal messaging, including calling patients to give test results, relay information from providers and make appointments.
  • Prepare the exam rooms in the provider’s area for the treatment of patients, making sure that each exam room is adequately stocked with supplies needed for the day.
  • Empty bins with confidential patient information and shred in accordance with proper confidentiality procedures.
  • Fully and accurately complete pre-visit planning prior to the physician or advanced practice provider seeing the patient.
  • Respond to incoming patient/customer calls, triage by acuity and patient needs to handle requests and medical needs.
  • Provide patient education and direction as needed.
  • Update patient care plans and reporting measures.
  • Reconcile medications, enter medication orders per provider’s instruction, refill medications per clinic policy.
  • Accurately enter provider orders in the EMR and carry out to completion.
  • Monitor and manage health care maintenance for patients, including identification of gaps in care, ordering screening tests and administering appropriate vaccinations.
  • Call patient back to the provider’s area and perform check-in procedures per protocol, including obtaining vital signs.
  • Prepare the patient for the physician or advanced practice provider’s exam.
  • Maintain a clean and tidy work station and exam room.
  • Perform in house procedures such as IV fluid administration, Nebulizer treatments, injections, EKG, eye exams, ear wash and other tasks as needed.
  • Maintain patient confidentiality and follow HIPAA guidelines.
  • Other duties or specialty jobs as assigned.

EDUCATION AND TRAINING REQUIREMENTS:

  • Diploma, Associates, or Bachelor’s degree from an accredited school of nursing required.
  • Minimum 1 year of LPN/RN/Medical Assistant experience preferred.
  • Must possess a current and valid Nebraska License.
  • Must possess a current and valid Basic Life Support (BLS).

OTHER SKILLS AND ABILITIES:

  • Is familiar with medical terminology and will use the Family Practice Abbreviation list correctly.
  • Planning and time-management abilities which facilitate the work flow of the physician or advanced practice providers.
  • Possess knowledge of computers and their function in the medical work place.
  • Safely operate necessary medical equipment required to perform Essential Job Duties.
  • Ability to perform a variety of duties, often changing from one task to another.
  • Ability to perform with frequent interruptions and/or distractions.
  • Respond appropriately to changing situations and patient needs.
  • Ability to recognize an emergency situation and take appropriate action.
  • Ability to establish and maintain working relationships with Family Practice Employees and the public.

WORK ENVIRONMENT: Possibility of exposure to biohazard materials and bodily fluids.

Job Type: Full-time

Pay: $21.00 - $35.00 per hour

Expected hours: 36 per week
