The Elementary Intensive Case Manager (EICM) provides case management services to children and families. The purpose of services will be to provide children and family focused intensive case management services to divert children from chronic truancy and detour entry into the delinquent behaviors.
The Elementary Truancy Specialist will provide intensive case management services for children ages 6 -12 years old and who are currently in Fuguitt and Ponce DeLeon Elementary Schools between 1st and 5th grade. Services are designed for children who are experiencing truancy at any level or at risk of significant truancy due to social/emotional issues, family discord, socio-economic issues or poor social integration.
Essential Duties/Responsibilities (Full-time 40-hour work week):
· Acts in accordance with the Mission, Vision, and Values of Family Resources.
· Conduct themselves in a manner supporting of a healthy culture within the agency.
· Provides services to youth during traditional and non-traditional business hours in communities, homes and/or schools and connects youth and families to a coordinated, comprehensive array of services that meet their ongoing needs.
· Must provide intensive case management services to youth in Elementary Schools; ages 6-12.
· Meets contract requirements of providing services to 32 children and families.
· Must work with school social workers and counselors to receive referrals.
· Completes face-to-face intake for each youth.
· Enters youth information into Apricot.
· Conducts a minimum of four direct contacts per month. Direct contact is defined as face to face contact with youth, parent or guardian for a minimum of 15 minutes per contact
· Establishes referral needs and assists in making referrals to identified services/agencies.
· Monitors youth and family's progress in services and utilization of services.
· Maintains complete, accurate and comprehensive records updated with information recorded in a timely manner.
· Other duties as assigned.

· Must demonstrate awareness and acceptance of cultural differences of clients, staff, and community contacts.
· Must have the ability to develop and maintain effective working relationships with individuals from a variety of cultural and ethnic backgrounds, including clients, staff, professional and community persons.
· Must possess excellent organizational skills, well developed verbal and written communication skills, and the ability to effectively deal with complex issues.
· Must maintain a high degree of professionalism and reliability within areas of responsibility by providing leadership, direction, motivation, and training.
· Should possess characteristics that show sound judgment in fulfilling position duties and responsibilities as well as a cooperative attitude in working relationships with senior members of management.
· The possession of a personal cellular phone is required for use in accordance with agency policies and procedures.
· Must have valid Florida Driver's License and auto insurance for personal vehicle and a clean driving history.
Education and Experience:
· Bachelor's degree from an accredited university with emphasis in areas of psychology, social work or related field.
· Minimum of two years experience working with youth and their families.
· Demonstrated capacity to provide case management services.
· Well versed in strengths-based programming, positive youth development and trauma-informed care service models.
Physical Requirements:
