ESSENTIAL FUNCTIONS :
- Work with clients and families to gather and complete NOMs questionnaires and gather other relevant data at intake.
- Submit NOMs data for database entry in a timely manner.
- Complete required client documentation at intake.
- Maintain and update client’s treatment plan and other paperwork as needed / appropriate
- Collaborate with other team members regularly.
- Assure that patient information is communicated to key staff prior to and after appointments.
- Confirm that patient has all required information to complete next steps in treatment
- Facilitate collateral communication between patients, support staff, direct care providers, and various school and community providers to assist patients in meeting all health-related goals.
- Active and timely participation in all administrative and treatment team meetings for purpose of case review, case assignments, and individual planning. Care Navigators’ caseloads shall be assigned and monitored by appropriate team leaders.
- Documentation is timely, current, and accurately reflects the services being provided and the patient’s response to those services.
- Act as an advocate for children / youth and families. This includes identifying gaps and developing proposals for meeting identified needs and accessing needed resources and services.
- Maintain collaborative relationships between agency departments and community resources. Participate in internal and external networking activities designed to increase the likelihood of meeting the client and family's identified needs.
- Complete required training for position.
- Respect and demonstrate ability to maintain client confidentiality.
- Act as an ambassador of Family Service and Guidance Center in the community setting.
- Support and actively promote the vision and mission of Family Service and Guidance Center and the philosophy of the strengths perspective.
- Other duties as necessary or required for the fulfillment of the CCBHC program, SAMHSA grant, or other strategic initiatives.
