ESSENTIAL DUTIES AND RESPONSIBILTIES

Intake
- Responsible for gathering and compiling intake information for clients interested in agency services in the Therapy Service program. Responsible for assigning clients to appropriate FSA staff, collecting necessary paperwork and opening new client charts.
- Responsible for directing clients to crisis services (CARES line, 988, walk-in hours) if the client presents with a more immediate need or safety concern
- Provide clients with information on the agency’s sliding fee scale and assist them in understanding payment options
- Stay up-to-date about different funding sources, including township-funded clients, and ensure clients receive appropriate information regarding their eligibility
- Responsible for responding to requests for information regarding community resources.
- Responsible for entering data into computer related to intakes and opening of charts in the Therapy Service programs and managing related paper work.
- Responsible for transferring of cases across programs, and responsible for all paperwork and data entry related to these transfers.
- Responsible for creating new client profiles in Greenspace and sending initial assessments to clients and guardians for baseline data.
- Responsible for verifying client information on the Medi system for each Therapy Service client and entering this into the client data base system.
- Responsible for verifying third party liability.
Maintaining Accurate Wait List
- Responsible for compiling and maintaining daily information on agency waitlist for Therapy Service programs and assigning clients from the waitlist to appropriate FSA staff in a timely manner.
- Responsible for weekly wait list reports to supervisor.
- Identifying trends in waitlist clients and providing information to supervisor, and provide a report to Director of OTP and CCO with the breakdown of waitlist numbers by the 4th week of the month.
Case Closings
- Responsible for closing clients if they do not engage in therapy services or have requested to be removed from the waitlist
- Responsible for maintaining and tracking closed files.
Administrative
- Responsible for attending staff meetings when requested.
- Responsible for attending Clinical Staff meetings as requested by supervisor.
- Responsible for serving on agency and/or program committees as requested by supervisor.
- Responsible for completion of and meeting deadlines for all agency administrative paperwork, including 1009’s, billing slips, time sheets, expense sheets and any other administrative or agency reports as requested.
- Maintain awareness of therapy and support groups offered within the agency and share relevant information with clients to ensure they are informed of available resources.
Supervision
- Responsible for informing supervisor of any problematic or emergency situations.
- Responsible for coordinating paperwork flow for appropriate supervisory signature.
- Responsible for meeting with supervisor on a regular basis.
Community Education/Public Relations
- Attend and participate in community education and public relations activities as requested by supervisor.
Other Duties/Special Assignments
- Perform other duties as assigned by supervisor.
- Provide front desk coverage when requested.
- Participate in agency Risk Management and Performance Quality Improvement activities.
- Perform incidental business related travel as needed.
- Perform job responsibilities with a positive attitude.
- Responsible for promoting collegiality at the workplace by working collaboratively with others.

PHYSICAL AND ENVIRONMENTAL CONSIDERATIONS
- The incumbent will be sitting in car for travel for several minutes every day. The incumbent is frequently expected to walk, sit, stand, bend and drive. The incumbent may also need to climb stairs on occasion. Must be able to lift up to 35 lbs. on occasion.
- The work environment is mostly field based and will have varying temperatures and noise level. The incumbent can expect to work in client’s homes, schools, public libraries, community agencies, and some in office time.

Hourly Pay Rates
- In accordance with Illinois state law, we are committed to transparency in compensation. The hourly pay range for this position is $20 to $24.38 per hour, depending on experience and qualifications

TRAVEL
This position requires light travel.
EDUCATION AND EXPERIENCE
- Must have a high school diploma or equivalent
- Must be at least 21 years of age and able to pass an extensive background check
- Must have excellent computer skills and be flexible
- Must be bilingual in English and Spanish
- Must be available Monday to Friday, 9AM to 5PM
