What are the responsibilities and job description for the Case Manager position at Family Service Communities?
- Assess resident’s physical and mental well-being in a compassionate and professional manner.
- Provide emotional support and interventions to resident and family members and develop a plan for resident to enhance resident quality of life.
- Provide resident with information, referrals and linkage to community agencies. Coordinate resident care needs with community providers to ensure resident needs are being met.
- Assist resident with accessing income entitlements and public benefits
- Arrange for client services and medical care to maintain optimal resident health.
- Provide discharge planning and assist with alternate living arrangements
- Maintain up to date documentation and client records. Documentation includes monthly assessments, case management and functional assessments and medical assessments
- Other duties as assigned
