SUMMARY
Family Service League is seeking a full-time Licensed Practical Nurse (LPN) for the Assertive Community Treatment (ACT) program, which provides behavioral health services for adults who have been diagnosed with a severe and persistent mental illness. The ACT program is comprised of a multi-disciplined team including skilled staff in psychiatry, nursing, and social work. The LPN will play an integral part of the team, working under the direction of the Registered Nurse the LPN will provide clients with medication management, health assessments and counseling. The Licensed Practical Nurse will be given an agency vehicle to use for ongoing home visits with the clients. Travel is required.

RESPONSIBILITIES
- The Licensed Practical Nurse will support the Registered Nurse in the development of individualized nursing screenings and interventions for consumers.
- Assist with the determination of medical care needs and other services to support wellness and consumer health.
- The Licensed Practical Nurse will facilitate consumer education on prevention, health, wellness, diet, smoking cessation, and recovery.
- Communicate with pharmacy and housing providers.
- Support and assist the Registered Nurse to ensure all consumers have primary care providers and receive annual physical and dental examinations.
- The Licensed Practical Nurse will administer IM injections.
- Monitor vital signs, side effects of medications, and report findings to the team.
- Assist the Registered Nurse with monthly medication sheets and annual health assessments.
- Complete daily progress notes and maintain nursing and health related data in EMR.
- The Licensed Practical Nurse will maintain and remain up to date with all ACT specific training.
- All other duties as assigned.
QUALIFICATIONS
New Grads Welcome!
Licensed Practical Nurse (LPN) with a current New York State License required.
Experience with the mental health population strongly preferred.
At least 2 years of experience working with adults with serious mental illness and co-occurring substance use disorders required.
Excellent time management, interpersonal, and verbal and written communication skills required.
Proficient computer skills, including Microsoft Office required; experience with an EMR is preferred.
Valid and clean NYS Driver’s License required.
