SUMMARY
Family Service League is seeking a part-time Case Manager for the Family Place Library. The Case Manager will provide linkages to resources, assistance with applications, and information to library patrons in the Brentwood Public Library.
RESPONSIBILITIES
- The Case Manager will conduct initial meetings with clients to determine needs.
- Assists clients in applying for public benefits.
- The Case Manager will provide linkage to mental health services when needed.
- Document client interactions on case forms.
- The Case Manager will obtain consent to share information where indicated.
- Translate letters written in English to Spanish for Spanish speaking clients.
- All other duties as assigned.
QUALIFICATIONS
High School Diploma or equivalent required. Bachelor's degree in Social Work or related filed preferred.
Minimum of 3 years of experience providing case management services to individuals and families required.
Excellent interpersonal and verbal and written communication skills required.
Proficient computer skills including Microsoft Office required.
Bilingual in Spanish required.
Valid and clean NYS Driver's License required.
PHYSICAL REQUIREMENTS
This position requires sitting for long periods of time.
