Job Overview:

At Family Service League Inc., we are seeking a dedicated Licensed Practical Nurse (LPN) to join our team in the Assertive Community Treatment (ACT) program. As an LPN, you will play a vital role in providing behavioral health services for adults with severe and persistent mental illness.

Key Responsibilities:

• Provide clients with medication management, health assessments, and counseling under the direction of the Registered Nurse.
• Maintain accurate records and update nursing and health-related data in the Electronic Medical Record (EMR).
• Facilitate consumer education on prevention, health, wellness, diet, smoking cessation, and recovery.
• Communicate effectively with pharmacy and housing providers.

Requirements:

• A current New York State License as a Licensed Practical Nurse (LPN) is required.
• At least 2 years of experience working with adults with serious mental illness and co-occurring substance use disorders is necessary.
• Excellent time management, interpersonal, and verbal and written communication skills are essential.
• Proficient computer skills, including Microsoft Office, and experience with an EMR are preferred.

• A competitive salary ranging from $55,000 to $55,000 per year.
