SUMMARY
Family Service League is seeking a full-time Case Manager for our Childrens Care Coordination Program in Bay Shore, NY. The Childrens Care Coordination Program is a home visiting program that offers care coordination for children and young adults with mental illness and / or chronic medical conditions. The Case Manager will be responsible for conducting home visits / meetings (both in-person and telehealth) with clients to develop strength assessments and individualized planning, providing linkages, assisting with navigating service systems, and supporting them with their goal achievements.
Evening hours are required for at least 2 days a week. Travel is required.

RESPONSIBILITIES
QUALIFICATIONS
Bachelors degree in social work and 2 years of relevant experience required OR Masters degree in social work and 1 year of relevant experience required.
Excellent interpersonal, verbal, and written communication skills required.
Proficient in computer skills, including Microsoft Office required.
Ability to work individually and as part of a team required.
Ability to multi-task and work in a fast-paced environment required.
Valid and clean NYS Drivers License.
PHYSICAL REQUIREMENTS
This position requires sitting for long periods of time and travelling within the community to conduct home visits.
