We are seeking a full-time Client Support Specialist for our Children's Care Coordination Program in Bay Shore, NY.
The Client Support Specialist will develop individualized planning to support children and young adults with mental illness and/or chronic medical conditions in achieving their goals.
The ideal candidate will have excellent interpersonal, verbal, and written communication skills, as well as proficiency in computer skills, including Microsoft Office.
The Client Support Specialist will be responsible for conducting home visits and assessments, providing linkages, and assisting with navigating service systems.

Responsibilities:
- Conduct outreach and engagement with clients with a primary history of mental illness.
- Develop strength's assessment with the client.
- Develop crisis/safety planning in collaboration with mental health providers.
- Meet a minimum of two times a month to support the youth with their goal achievements.
- Assist in the coordination and linkage of services with providers, benefit systems, and the community.
- Help clients navigate service systems to get the support they need in their recovery.
- Report incidents and accidents to the agency.
