Your primary focus will be on developing and implementing strategies to promote empowerment and self-advocacy among clients.
Key Responsibilities

- Develop and implement strategies to promote empowerment and self-advocacy among clients.
- Provide education and support to clients on managing chronic medical conditions and mental health issues.
- Coordinate access to community resources and services, including mental health and medical services.
- Monitor client progress and make adjustments to strategies as needed.
Qualifications
- Bachelor's degree in Social Work or related field required; Master's degree preferred.
- 2 years of experience working with youth and families in a social services setting.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Valid New York State Driver's License required.
