About the Job
The Case Manager role in our Children's Care Coordination Program involves working with children and young adults with mental illness and/or chronic medical conditions. The successful candidate will have a strong understanding of social work principles and excellent communication skills.
Duties and Responsibilities
- Meet with clients to assess their strengths and develop individualized plans.
- Provide linkages, assist with navigating service systems, and support clients in achieving their goals.
- Collaborate with family peer support workers and other providers within child/family treatment teams.
Requirements
- Bachelor's degree in social work and 2 years of relevant experience or Master's degree in social work and 1 year of relevant experience.
- Excellent interpersonal, verbal, and written communication skills.
- Ability to work independently and as part of a team.
- Valid and clean NYS Driver's License.
