At Family Service League Inc., we are seeking a dedicated Healthcare Professional to join our Assertive Community Treatment (ACT) program. As part of this multidisciplinary team, you will work closely with clients diagnosed with severe and persistent mental illness, providing essential support and care.
About the Role:
- The ideal candidate will possess a current New York State License as a Licensed Practical Nurse (LPN).
- A minimum of 2 years' experience working with adults with serious mental illness and co-occurring substance use disorders is required.
- This role involves providing medication management, health assessments, and counseling services to clients under the supervision of a Registered Nurse.
Responsibilities:
- Support the development of individualized nursing screenings and interventions for consumers.
- Assist in determining medical care needs and other services to promote wellness and consumer health.
- Facilitate consumer education on prevention, health, wellness, diet, smoking cessation, and recovery.
- Communicate effectively with pharmacy and housing providers.
- Ensure all consumers have primary care providers and receive annual physical and dental examinations.
- Administer IM injections, monitor vital signs, and report findings to the team.
