SUMMARY
Family Service League is seeking a full-time Intensive Home Worker for our Home Base program in Bay Shore. The Intensive Home Worker will work with children and young adults, between the ages of 5-18, who are severely emotionally disturbed and at risk. The Intrusive Home Worker will provide services that will help facilitate the emotional, social, and learning development of the children. The Intensive Home Worker will support the children and their families by implementing both their treatment plans (from their clinical provider) and their family development plans (established by the Home Base/CCSI program).

RESPONSIBILITIES
- The Intensive Home Worker will reinforce desired cognitive and behavioral changes to prevent crisis and to support the emotional well-being of the children and their families.
- Work with families in a culturally and linguistically responsive manner.
- Provide individualized assistance to families which may include anger management, socialization, psychoeducation, post crisis debriefing, re-enforcing the integration of safety plans, developing healthy coping mechanisms, interpersonal communication, etc.
- The Intensive Home Worker will provide services in the home or in the community at a frequency agreed upon by the children and their families.
- Communicate all crisis situations with the Program Coordinator and FDS.
- Work as a team with the FDS to support the children and their families.
- All other duties as assigned.
QUALIFICATIONS
Master’s Degree in Health and Human Services or a related field and 2 years of related experience OR Bachelor’s Degree in Health and Human Services or a related field and 4 years of related experience required.
Excellent interpersonal, verbal, and written communication skills required.
Proficient computer skills, including Microsoft Office required.
Bilingual in Spanish preferred.
Valid and clean NYS Driver’s License required.
