Job Title: Part-time Psychiatric Nurse Practitioner
Description: Family Service League is seeking a part-time salaried Psychiatric Nurse Practitioner for the Assertive Community Treatment (ACT) program. This program provides behavioral health services to adults with severe and persistent mental illness.
Responsibilities:
• Prescribe and revise medical regimens
• Interpret findings and plans of care according to collaborative practice agreements and protocols
• Provide medical assessments and vitals for consumers in the community
• Conduct health assessments of adults with potential, acute, and/or chronic health problems
• Initiate referrals to specialty services and foster continuity of care
• Educate patients and their family members on health promotion, maintenance, and disease prevention
• Participate in team meetings and conferences to enhance interdisciplinary primary health care delivery
• Serve as liaison between patient and community, and act as health care advocate
• Identify quality improvement issues and document standards of care and practice
• Act as resource to staff and implement educational programs
Requirements:
• At least 1 year of clinical experience in a behavioral health setting
• Proficient computer skills, including Microsoft Office
• Ability to sit for long periods and travel to conduct home visits
