Responsibilities:
- Prescribe and revise medical regimens according to established protocols.
- Interpret findings and plan care collaboratively with other healthcare professionals.
- Conduct medical assessments and vital sign checks for consumers in the community.
- Perform comprehensive health assessments of adults with potential acute or chronic health problems.
- Refer patients to specialty services and promote continuity of care.
- Provide patients and their family members with education and counseling on health promotion, maintenance, and disease prevention.
- Participate in team meetings and conferences to enhance interdisciplinary primary healthcare delivery.
- Serve as a liaison between patients and the community, advocating for healthcare needs.
- Identify quality improvement opportunities and document standards of care and practice.
- Act as a resource to staff and implement educational programs.
Requirements:
- At least 1 year of clinical experience in a behavioral health setting is required.
- Proficient computer skills, including Microsoft Office, are necessary.
