Job Description:
We are seeking a highly motivated Peer Specialist to join our multidisciplinary team at Family Service League Inc. The successful candidate will be responsible for providing outreach support and coordination of care for clients in our Certified Community Behavioral Health Clinic (CCBHC) in Riverhead.
About the Role:
- As a Peer Specialist, you will work closely with clients to facilitate their engagement in services and provide additional support to help them achieve their goals.
- You will provide coaching and recovery support, including issues of self-advocacy, linkage to aftercare, wellness self-management, medication adherence, smoking cessation, entitlements, and daily living activities.
- You will accompany clients to medical and other appointments to ensure they receive the support they need.
- As a resource to all team members, you will provide expert advice on psychosocial and substance abuse issues and resources.
Responsibilities:
- Provide outreach and engagement services to clients, including telephonic and face-to-face contact.
- Develop and maintain relationships with community resources and service providers to ensure seamless care transitions.
- Maintain accurate records and statistics, ensuring timely and organized reporting.
- Collaborate with healthcare professionals to coordinate care planning and provide expert input on client needs.
Qualifications:
- A High School Diploma or equivalent is required.
- Prior experience as a peer specialist in a related setting is preferred.
- The ability to draw upon personal experience living with a disability and/or social/emotional challenge is essential.
- Familiarity with NYS Entitlements, Housing, Health, and/or Mental Health/Substance Abuse Service Delivery System is an asset.
- Excellent interpersonal and verbal and written communication skills are required.
- Proficient computer skills, including Microsoft Office and Electronic Health Records, are necessary.
